What else does an SMO’s job?
An SMO is a low brace. It stops just above the ankle bones and deliberately leaves the ankle free to bend,1 which means what it really controls is side-to-side heel position and not much else. Every sensible SMO alternative is picked by answering one question first: what are you actually trying to control? And the swap families ask about most often is not an alternative at all.

These are two unrelated questions that use the same vocabulary, and mixing them up is where families lose ground.
If you are in Ponseti maintenance, an SMO is not the alternative
After casting, the correction is held by a brace that fastens both feet to a bar and holds them turned outward.2 That held rotation is the active ingredient. An SMO holds one foot at a time, does not rotate it, and has no connection to the other side, so it cannot do that work no matter how well it is made.
This matters more than anything else on the page. The abduction brace is the part of Ponseti treatment with the clearest link to keeping a correction,2 and swapping it for something a child tolerates better is the most common way families lose what casting achieved. If the brace is the problem, the fix is a bracing conversation, not a different device bought around it.

The range
Less control to more, and what each one gives up
Orthoses sit on a spectrum. Moving down it buys comfort and bulk; moving up it buys control. Nothing on this list is better in the abstract.
Supportive footwear on its own
A firm heel counter and a shoe that resists twisting does more than people expect, and for a mild, comfortable foot it is sometimes the whole plan. It gives up any direct hold on the heel.
A custom foot orthosis
An in-shoe device shaped to the foot. Lower profile than anything else here, invisible from outside, and the easiest to live with. It supports the arch and cushions load, but it does not grip the heel, so a heel that rolls hard will keep rolling.
UCBL
A rigid in-shoe device with walls that wrap the heel and hold it upright. This is the nearest thing to an SMO that still disappears into a shoe, and it is the usual answer when the complaint is bulk rather than control. It gives up the leverage an SMO gets from sitting higher.
SMO
Trims just above the ankle bones. Controls the heel side to side with more leverage than a UCBL, and leaves the ankle free to bend.1 The trade is a wall around the ankle, a warmer foot, and a shoe that has to be deep enough to take it.
DAFO
A thin, flexible ankle-foot orthosis that crosses the ankle. Adds some control over how far the ankle bends while staying lighter than a rigid brace. Chosen when heel control alone is not holding the position.
AFO
Crosses the ankle with real rigidity and controls dorsiflexion and plantarflexion as well as the heel.1 The step taken when the problem is the ankle itself, not the heel underneath it. Bulkiest, hardest to fit into shoes, and the most control available short of surgery.
Better question
Start from the complaint, not the device
Most people searching for an SMO alternative have a specific problem, and several of them are solved by changing the SMO rather than replacing it.
It rubs at the ankle bone. Usually a trim line or padding issue, and adjustable. Wear and care guidance is explicit that redness which does not fade within about twenty minutes of removal needs looking at rather than tolerating.4 A different device is the last step here, not the first.
Nothing fits over it. A shoe problem, not a brace problem, and the shoe is far easier to change. Depth, a firm counter and a removable footbed are what make the difference.
It is too hot, or too obvious. A real reason to ask about a UCBL. This is the complaint most likely to end in a genuine step down the range.
My child fights it every morning. Worth separating from everything above. Tolerance problems and control problems have different fixes, and a smaller device that gets worn can beat a better device that does not.
It does not seem to be working. The one complaint that should go straight back to the prescriber. It may mean too little control rather than too much, in which case the answer is up the range, not down it.

What the evidence does and does not settle
There is no clean comparative answer waiting to be found. Head-to-head trials of these devices in children are limited, and prescription rests substantially on what an examination finds in the individual foot: how much motion there is, where it happens, and how the child loads it.1 Anyone offering a confident ranking is describing a preference.
What is well established is narrower and more important: the abduction brace maintains a Ponseti correction, and the risk of losing that correction is bound up with whether the brace is worn.23 That is a different question from which orthosis suits a foot day to day, and it is the one where substituting a device is genuinely costly.
Worth asking
Four questions for the orthotist
What is this SMO controlling, specifically? The answer names a movement and a part of the foot. If it does, you can ask sensibly whether something lower down the range still covers it.
What would we lose by stepping down to a UCBL? A direct question with a direct answer, and the one most likely to produce a workable compromise.
Is this holding a correction, or supporting a position? Holding a correction is a much harder thing to hand to a smaller device.
What would tell us it is not enough? Gives you something to watch for rather than a vague instruction to come back.
